Reprinting CMS 1500 Forms
This topic explains how to reprint CMS 1500 forms in AcuityLogic Billing. You can reprint a CMS 1500 form only after the claim has been billed.
Printing a CMS 1500 form marks the claim as Billed. Reprinting the CMS 1500 form does not change the status of the claim.
To reprint a CMS 1500 form
- Search for a billed claim whose CMS 1500 form you want to reprint. See Searching for Claims.
- From the Action drop-down list of the claim whose CMS 1500 form you want to reprint, select one of the following:
- Reprint CMS Form (Version 08/05)
- Reprint New CMS Form (Version 02/12)
