Printing the Minor Consent Form in Book View in AcuityLogic POS

If you schedule an appointment for a patient who is 17 years old or younger, the Print Minor’s Consent (M) icon displays in Book View.

To print the minor consent form in Book View

1. Open Book View to the Manage Appointments view. See Navigating Book View in AcuityLogic POS.
2. Click the Print Minor’s Consent icon () in the Actions column for the appointment.

   The Minor’s Consent form opens in a new window.
