Recording Additional Patient Information
From the Other Information window, you can record additional patient information including demographic information, additional addresses, responsible party information, and dependents. This section explains how to record additional patient information.

- Open a patient profile. See Opening Patient Profiles.
- Click Other Information.
The Other Information text displays in green after you add details in the Other Information window.
The Other Information window opens.
- Open the Other Information window. See Recording Additional Patient Information.
- In the Demographic Information section, select one or more check boxes for the patient’s race from the Race drop-down list.
- Select the patient’s ethnicity from the Ethnicity drop-down list.
- Select the patient’s preferred language from the Preferred Language drop-down list.
- Click Save to save the patient’s demographic information.
- Open the Other Information window. See Recording Additional Patient Information.
- In the Additional Addresses section, click Add New Address.
- Enter or select the patient’s additional address information in the fields highlighted in yellow.
- Click Save to save the address information.
- The responsible party, also known as a guarantor, is the person responsible for paying the patient’s account bills.
- Only a patient with an existing patient profile in your office can be selected to be the responsible party for another patient.
- Family members typically have the same guarantor.
- Recording responsible party information for a patient does not combine patient balances for multiple patients. AcuityLogic tracks the patient balance for each patient separately.
- Open the Other Information window. See Recording Additional Patient Information.
- In the Responsible Party section, click Change Responsible Party.
- Enter the name of the responsible party in the Last Name and First Name fields.
- Click Save.
- Open the Other Information window. See Recording Additional Patient Information.
- In the Dependents section, click Add Dependent.
- Enter or select the dependent’s name in the Last Name and First Name fields.
- If the dependent is a new patient, record the middle initial, birth date, title, and gender for the dependent in the provided fields.
- Use the Yes or No radio buttons to select the patient’s information to copy to his or her dependent.
- Click Save.
AcuityLogic creates a patient profile if the dependent is a new patient.
If you link contact and insurance information to a dependent and later modify the patient’s information, AcuityLogic does not update the information in the dependent’s profile. You must manually update the dependent’s information.
If you add a dependent to a patient, AcuityLogic lists the patient as the responsible party for the dependent in the Other Information window of the dependent’s profile.
