Searching for a Patient’s Previous Name

If you have trouble finding a patient’s records, the patient’s name might have changed since his or her information was entered in your system. If the name was changed in the patient’s profile, managers can conduct a Previous Patient search in AcuityLogic POS to see whether the patient has records under both the old and new names.

To search for a patient’s previous name

  1. Open the Management window. See Opening the Management Window.
  2. Click the Previous Patient tab.
  3. In the tab, enter part or all of the patient’s previous last name, previous first name, or both names.
  4. Click Search.

    The search results include the following information:

    • Current Name: The name to which the original name was changed
    • DOB: The birth date associated with the current name
    • Note Text: Any names matching your search criteria that were changed to the current name
    • Date Changed: The date the name change occurred
