Analytics Data Dictionary

This topic covers how the various data points across each report are determined.

Keep in mind that the data reflected in Analytics only comes from what you've documented in Eyefinity EHR, not the practice management side of the platform. In addition, data from Eyefinity EHR only appears in Analytics for visits you've finalized in the last three years. You won't see any data from preliminary visits or visits older than three years.

Data points are separated into two categories:

Brand Name

The branded name of the associated medication

CD Ratio

The cup-to-disc ratio (CD ratio) measurement documented on the associated visit

Chief Complaint

The chief complaint documented on the associated visit

Code Order

The position the associated CPT code appears on the encounter form

Co-management Additional Information

The information documented in the associated Co-management plan

Comanagement CPT Code

The CPT code documented in the associated Co-management plan

Comanagement Detail Level

The detail level of the associated Co-management plan

Comanagement Relinquish/Assumption of Care

Indicates if the provider is relinquishing care or assuming care on the associated Co-management visit

The CPT code(s) documented on the associated encounter form

CPT Category

The category of the associated CPT code

CPT Description

The description of the associated CPT code

CPT Unit

The number of CPT units documented on the associated encounter form

Date/Time Sent

The date and time the associated outbound fax was sent, in MM/DD/YYYY format

Diagnoses

The names of all diagnoses on the associated visit, concatenated

Diagnosis Name

The name of each individual diagnosis on the associated visit

Diagnosis Status

The status of the associated diagnosis (e.g., New, Improving, Resolved)

Diagnosis Type

The diagnosis type of the associated diagnosis

Dose

The dose of the associated medication that was prescribed or recorded in the patient's clipboard

Established Patient

Indicates if the current status of the associated patient is established

Extraction Method

The type of extraction method documented on the associated cataract visit

Facility

The name of the facility on the associated visit

Fax Facility

The facility documented on the associated outbound fax

Fax Recipient

The individual or facility the associated outbound fax was sent to

Fax Recipient HISP Address

The HISP address of the facility the associated outbound fax was sent to

Fax Recipient Specialty

The specialty of the individual or facility the associated outbound fax was sent to

Fax Status

The current status of the associated outbound fax

Fax Template

The template of the associated outbound fax

Finalized Date/Time

The date the associated visit was finalized, in MM/DD/YYYY format

Follow Up Reason

The follow up appointment reason documented on the associated visit

Follow Up Time

The follow up appointment time documented on the associated visit

Form

The form of the associated medication that was prescribed or recorded in the patient's clipboard

Frequency

The frequency of the associated medication that was prescribed or recorded in the patient's clipboard

Generic Name

The generic name of the associated medication

ICD-10

The ICD-10 code(s) documented on the associated encounter form

ICD-10

The ICD-10 code(s) documented on the associated encounter form

IOP

The intraocular pressure (IOP) documented for the associated patient

IOP Method

The method of obtaining the intraocular pressure (IOP) documented for the associated patient

Intraocular Lens Used

The type of lens documented on the associated Cataract visit

Last Billing Provider

The name of the billing provider on the associated patient's most recent visit

Last Facility

The name of the facility on the associated patient's most recent visit

Last Primary Provider

The name of the primary provider on the associated patient's most recent visit

Medication Name

The name of the associated medication that was prescribed or recorded in the patient's clipboard

Medication Status

The current status of the associated medication that was prescribed or recorded in the patient's clipboard

Medication Type

The brand type of the associated medication (e.g., Brand, Generic)

Modified by User

The user who modified the associated Office Flow room

Modifiers

The modifier(s) documented on the associated encounter form

Morphology

The clinical description of the associated diagnosis

New Dx?

Indicates if the associated diagnosis is a new diagnosis

Ocular History

The ocular history documented in the associated patient's clipboard

OD/OS

Indicates if the associated value represents the right eye (OD) or left eye (OS)

Order Date

The date the associated order was created

Order Diagnosis

The diagnosis on the associated order

Order Due Date

The date the associated order is marked as due in MM/DD/YYYY format

Order Name

The name of the associated order

Order Number

The unique number of the associated order

Order Performed at Location

The location the associated order is to be performed at

Order Priority

The priority of the associated order

Order Scheduled Date

The date the associated order is scheduled to be performed in MM/DD/YYYY format

Order Type

The order type of the associated order

Order Status

The current status of the associated order

Original CPT

The CPT code that was suggested by Eyefinity EHR on the associated encounter form

Override Higher/Lower

Indicates if the associated E/M CPT code was overridden to a higher E/M CPT code or a lower E/M CPT code

Overridden CPT

The CPT code that was manually entered on the associated encounter form

Past Medical History

The medical conditions documented in the associated patient's clipboard

Patient Age Decile

The age of the associated patient, grouped in 10-year increments

Patient Cell Number

The mobile number of the associated patient

Patient City

The home city address of the associated patient

Patient Created By

The user who manually created the associated patient's chart

Patient DOB

The date of birth of the associated patient

Patient Email

The email address of the associated patient

Patient Ethnicity

The ethnicity of the associated patient

Patient Gender

The gender of the associated patient

PGA

The Physician's Global Assessment (PGA) documented on the associated visit

Pharmacy Name

The name of the pharmacy documented in the associated patient's chart

Pharmacy Phone Number

The phone number of the associated pharmacy

Pharmacy Release By

The user who electronically sent the associated medication to the pharmacy

Pharmacy Released Date

The date the associated medication was electronically sent to the pharmacy in MM/DD/YYYY format

Plan Name

The name of the plan on the associated visit

Plan Type

Indicates if the associated procedure for Macular Degeneration was an injection procedure or laser procedure

Portal Username

The username of the associated patient's portal

Preferred Primary Provider

The name of the preferred primary provider, as documented in the associated patient's chart

Prescriber

The user who prescribed the associated medication

Primary Care Provider

The name of the primary care provider, as documented in the associated patient's chart

Primary Provider

The name of the primary provider on the associated visit

Primary Referral Source?

Indicates if the referral source is the primary referral source documented on the associated patient chart or visit

Procedure Name

The name of the procedure documented in the associated patient's procedure log

Provider Non-NPI

The non-NPI number of the associated provider

Provider Role

The role of the associated provider

Punctal Occlusion Type

The type of procedure documented on the associated Dry Eye Syndrome visit

Referral Source Name

The name of the referral source associated to the patient or visit

Referral Type

The type of referral source associated to the patient or visit

Referring Provider City

The city address of the associated referring provider, as documented in the associated patient's chart

Referring Provider Fax Number

The fax number of the associated referring provider, as documented in the associated patient's chart

Referring Provider HISP Address

The HISP address of the associated referring provider, as documented in the associated patient's chart

Referring Provider Mobile Number

The mobile number of the associated referring provider, as documented in the associated patient's chart

Referring Provider Name

Written on Date

The date the associated prescribed medication was written in MM/DD/YYYY format

Average PGA

The sum of Physician Global Assessment (PGA) scores divided by the total number of PGA scores

Average Time

The average time it has taken to finalize visits for the associated provider

Average Time Spent in Room

The sum of the time spent in all rooms divided by the total number of Office Flow rooms

Bad Login Count

The total number of unsuccessful log in attempts to the patient's portal

Completed Task Count

The total number of tasks currently in the Completed status

Consult CPT %

The percentage of patients that were billed a Consult CPT code

CPT Count

The total number of CPT codes, including units

Established CPT %

The percentage of patients that were billed an Established Patient CPT code

Established Patient Count

The total number of patients currently with a patient status of Established

Fax Count

The total number of faxes received into or sent out of the practice

Good Login Count

The total number of successful log in attempts to the patient's portal

Medication Count

The total number of medications that were prescribed or recorded in patients' clipboards

Modifier 25 %

The percentage of visits with a documented 25 modifier

Modifier 59 %

The percentage of visits with a documented 59 modifier

New Patient Count

The total number of patients currently with a patient status of New

New Patient CPT %

The percentage of patients that were billed an New Patient CPT code

New Task Count

The total number of tasks currently in the New status

The total number of visits that contain manually overridden charges

Page Count
