Documenting Contact Lens Follow Ups
There are several ways that you can document a contact lens follow up in Eyefinity EHR.
Depending on your preference, you'll want to do one of the following:
- Document the follow up using a previous diagnosis. This is the quickest method to document a contact lens follow up when the patient has come in for a visit.
- Document the follow up using a new chief complaint. This is the most-detailed method for documenting a contact lens follow up when the patient has come in for a visit.
- Document the follow up using a non-visit order. If the patient has called in because they like their trials and want to order a supply, you can use this method to quickly finalize their trial Rx without creating a new visit.

Create a new visit for the patient.
The Visit Overview opens.
- Click the CC/HPI/ROS button on the action bar.
- Select the check box for the diagnosis you're following up on from the Follow Up CC window.

Click Save and Continue.
A list of questions appears for the follow up.

Under The patient presents for, select Further Evaluation and Management.
Optional: Answer any other questions as needed.

Click Continue.
The follow up chief complaint is created.
- Click the Vision Exam button on the action bar.
- Click the Wearing (WRx) tab.

Click the Contact Lenses tab underneath the action bar.
If the patient is wearing an RGP, you'll need to click the RGP Contact Lenses tab and then click New RGP Wearing.
- Click the Refraction History button.
- Click Import for the trial lens the patient is currently being evaluated for.
- Under status, select Dispensed Successful or Dispensed Unsuccessful depending on whether or not the trial lens was successful for the patient.
- Click Save.

If the lens was successful for the patient, click Create Final Rx on the trial lens.
If the lens was unsuccessful, click the Soft Contact Lenses or RGP/Hybrid Contact Lenses tab and document the next trial lens the patient will be trying out.
- Click Visit Note on the action bar to return to the Visit Overview.
- Click Finalize Visit.

Create a new visit for the patient.
The Visit Overview opens.

Click the CC/HPI/ROS button on the action bar.
The chief complaint selection screen appears, where you can search for a chief complaint.
- In the search bar, begin typing "Contact Lens Detailed".

Select Contact Lens Detailed from the drop-down list.
The chief complaint details screen appears.

Under "Is this a new presentation, or a follow-up?", select the check bubble for Follow Up Contact Lens Evaluation.
Optional: Answer any other questions as needed.
- Click Save.
- Click the Vision Exam button on the action bar.
- Click the Wearing (WRx) tab.
- Click the Contact Lenses tab underneath the action bar.
- Click the Refraction History button.
- Click Import for the trial lens the patient is currently being evaluated for.
- Under status, select Dispensed Successful or Dispensed Unsuccessful depending on whether or not the trial lens was successful for the patient.
- Click Save.

If the lens was successful for the patient, click Create Final Rx on the trial lens.
If the lens was unsuccessful, click the Soft Contact Lenses tab and document the next trial lens the patient will be trying out.
- Click Visit Note on the action bar to return to the Visit Overview.
- Click Finalize Visit.
- Open the patient's chart.
- Click the New Non-Visit Order button on the action bar.

In the Note box, type a reason for the non-visit order, such as:
"Patient called and wants to order trials. OK to order per doctor."
- Click Save Note.
- Click the Glasses and Contacts button on the action bar.
- Click the Trials tab.

Click New Soft Contacts for the trial you want to create a final Rx for.
Optional: Change the status to Final by selecting it from the drop-down list.

Click Save.
Optional: Print the final contact lens Rx for the patient.
- Return to the order note overview.
- Click the Finalize Order button on the action bar.
- Click Finalize Note to confirm.
