Managing Medical History Questions
For clinical staff, you can choose which medical conditions and surgeries appear by default within the patient's Clipboard. This enables you to reduce the number of questions the provider or tech must scroll through during the patient history. However, clinical staff can search for and add conditions and surgeries to an individual patient's Clipboard as needed to complete the patient's history. Any questions previously answered on the patient Clipboard will still remain visible no matter which conditions and surgeries are hidden.
What do you want to do?- Log in to the Eyefinity EHR web application as an administrator.
- Click the Clipboard/Kiosk/Portal Settings tab at the top of the page.

Locate and expand the Patient Clipboard, Kiosk, and Portal Settings section and click the Manage Sections button.
- Click the Past Medical History tab on the left side of the page.
- Navigate to the Past Medical History tab. For more information, go to Open the past medical history settings.
- For each medical condition and surgery you've enabled, click the Patient Term Description drop-down menu select the description that patients see within the portal and kiosk.
- Click Save to apply your changes.
You can now display specific medical history questions based on the patient's age and gender. This enables you to hide questions from the patient portal and kiosk that may not apply to the individual patient. These settings also impact which questions clinical staff see in the patient's clipboard. However, any questions that were previously answered are still visible even if they no longer apply to the patient.
- Navigate to the Past Medical History tab. For more information, go to Open the past medical history settings.
- To limit a question to a specific gender, select the Gender.
The default setting is "Both," which displays the question to all patients.
- To limit a question based on the patient's age, enter an Age Range.
The age range is inclusive of the numbers entered at the top and bottom of the range. For example, a question with an upper age range of 50 will appear until the patient's 51st birthday.
Any active questions that don't have age or gender limits will be visible to all patients.
- Click Save to apply your changes.
- Navigate to the Past Medical History tab. For more information, go to Open the past medical history settings.
- For each medical condition or surgery, select the Clipboard and Portal check box to display the condition to clinical staff in the Clipboard and to patients in the portal. Deselect the check box to hide the condition or surgery from everyone.
- For each medical condition and surgery you've enabled, select the New and Existing check boxes to display the condition or surgery to new and established patients within the kiosk.
When you deselect the Clipboard and Portal check box, the condition or surgery is hidden from the patient Clipboard, the patient portal, and the kiosk. However, clinical staff can search for and add conditions and surgeries to an individual patient's Clipboard as needed to complete the patient's history.
Conditions and surgeries that display a lock icon rather than a check box are required for legal or regulatory reasons and cannot be hidden.
- Click Save to apply your changes.
- Navigate to the Past Medical History tab. For more information, go to Open the past medical history settings.
- Type a condition name or SNOMED in the Search to Add field.
- Select the condition from the list of available matches.
If the condition isn't already listed, it's appended to the bottom of the list.
- Select a Patient Term Description and Gender and Age filters as needed. For more information, go to Hide questions based on patient age and gender.
- Select the Clipboard and Portal and Kiosk Patients check boxes as needed. For more information, go to Manage where medical history questions appear.
- Click Save to apply your changes.
- Navigate to the Past Medical History tab. For more information, go to Open the past medical history settings.
- Locate the medical condition or surgery and click the Delete icon in the right column.

Click Save to apply your changes.
To restore a deleted condition or surgery, go to Add a new medical history question.
