Creating and Editing Waivers

The Eyefinity Kiosk enables you to electronically record the patient waivers that have been traditionally administered on paper during patient intake. These waivers may include a consent to receive medical treatment, release of patient information for billing and coordination of care, or agreement to pay.

Having patients sign waivers within the kiosk reduces the amount of paper office staff must manage and ensures patients are signing them.

Understanding Waivers

When waivers are enabled, the patient is presented with these documents in the kiosk first. The patient signs each document with a finger. A practice administrator can determine how frequently the patient is presented with each waiver, but once a year is the most common setting.

By default, if the patient declines to sign any one of the waivers, he or she is prompted to log out of the kiosk and may not proceed any further. An alert displays on the Patient Chart to notify clinical staff that the documents were not signed.

However, practice administrators can mark individual waivers as optional. If the patient chooses not to sign an optional waiver, he or she may continue to check in using the kiosk, and no alert displays in the Patient Chart to notify clinical staff the waiver was declined.

Creating and Editing Waivers

  1. Log in to Eyefinity EHR on the web as an administrator.
  2. Locate the Firm Consents group, and click Manage Patient Waivers.
  3. Click Add Patient Waiver to create a new waiver, or click Modify to edit an existing one.
  4. Enter the waiver Title.

    The title should concisely describe the waiver and will be visible to patients.

  5. Enter a Patient Waiver Description.

    This should be a plain language summary of the waiver and will be visible to patients.

  6. Enter the Patient Waiver Legal Description.

    This should be the legal text of the waiver and will be visible to patients.

  7. Click Save.
  8. Click the Frequency drop-down and select how often the patient should be required to sign the waiver.

    The most common selection is once per year.

  9. To make a waiver optional, select Can Be Declined.

    Optional waivers may be declined by the patient. Declined waivers are not saved i the patient's chart.
