012. Primary Open-Angle Glaucoma (POAG): Optic Nerve Evaluation
Percentage of patients aged 18 years and older with a diagnosis of primary open-angle glaucoma (POAG) who have an optic nerve head evaluation during one or more office visits within 12 months. The EHR reporting method also requires and optic disc exam for structural abnormalities.
This measure is to be reported a minimum of once per performance period for patients seen during the performance period. It is anticipated that eligible clinicians who provide the primary management of patients with primary open-angle glaucoma (in either one or both eyes) will submit this measure.
The 2027F Category II code has been retired. Quality Measure 12 (POAG: Optic Nerve Evaluation) is no longer available for claims-based reporting.
Document the optic nerve evaluation:
- Document a dilated eye exam.
- Document your findings on the Discs and Fundus tabs.
- Select a POAG Diagnosis.
- Select an optic nerve evaluation Plan. and tap Save.
- Document the findings.
- Tap Done.
- Document the rest of the visit and Finalize.
Alternatively, code quality with a special plan:
- Document a Diagnosis and Plan as you normally would.
- Tap Special Plans and select MIPS.
- From Popular Plans, select MIPS Quality and tap Save.
- Locate the MIPS Quality plan and tap Resume.
- Tap the POAG tab.
- Select an optic nerve evaluation outcome from the Quality 12 drop-down.
- Tap Done.
- Document the rest of the visit and Finalize.
