389. Cataract Surgery: Difference Between Planned and Final Refraction
Percentage of patients aged 18 years and older who had cataract surgery performed and who achieved a final refraction within +/- 1.0 diopters of their planned (target) refraction.
This measure is to be submitted each time for denominator eligible cases as defined in the denominator criteria. This measure is intended to reflect the quality of services provided for the patient receiving cataract surgery. This measure may be submitted by MIPS-eligible clinicians who perform the quality actions as defined by the numerator based on the services provided and the measure-specific denominator coding.
When surgery is recommended for the patient, perform the following steps:
- Document a cataract Diagnosis.
- Select the Optical Biometry Plan and tap Save.
- Locate the Optical Biometry plan you just added and tap Resume.
- On the Details tab, document the Targeted Refraction (Diopters).
- Tap Done.
- Document the rest of the visit and Finalize.
When the patient arrives for the postop visit within 90 days, perform the following steps:
- Document the patient's current refraction in the Vision Exam.
- Document a cataract or postop cataract Diagnosis.
- Select a postop cataract evaluation Plan and tap Save.
- Locate the Postop Evaluation Cataract plan you just added and tap Resume.
- On the Details tab, document the Postop Day.
- Tap Done.
- Document the rest of the visit and Finalize.
If the patient’s refraction is +/- 1.0 diopters of their planned refraction within 90 days of surgery, the patient will render as favorable.

Patients who achieved a final refraction (spherical equivalent) of +/- 1.0 diopters of their planned (target) refraction (spherical equivalent) within 90 days following cataract surgery. The refraction planned and final refraction values should correspond to the eye that underwent the cataract procedure.
