Adding and Editing Consent Forms

You can add or edit practice-wide consent forms as an administrator.

To add or edit a consent form

  1. Log into Eyefinity EHR as an administrator.
  2. In the Firm Consents section, click Manage Consents.
  3. Click Add Consent to create a consent form, or click Modify to edit an existing consent form.
  4. Enter the Title.
  5. Enter the text of the consent form under Consent Description.
  6. Enter the legalese for the consent form under Consent Legal Description.

    This is optional. If you don't want to enter legalese, you can type a period into the box.

  7. Add any signature lines you want to appear on the consent form.
  8. Click Save.
