117. Diabetes: Eye Exam
Percentage of patients 18–75 years of age with diabetes and an active diagnosis of retinopathy in any part of the measurement period who had a retinal or dilated eye exam during the measurement period or diabetics with no diagnosis of retinopathy in any part of the measurement period who had a retinal or dilated eye exam during the measurement period or in the 12 months prior to the measurement period.
This measure is to be submitted a minimum of once per performance period for patients with diabetes mellitus seen during the performance period. This measure may be submitted by Merit-based Incentive Payment System (MIPS) eligible clinicians who perform the quality actions described in the measure based on services provided and the measure-specific denominator coding.
Eyefinity EHR generates two sets of codes for 117. Diabetes: Eye Exam:
- When a provider documents M1220 through a diabetic counseling plan, a 2022F code will also populate as a line item on the bill.
- When a provider documents M1221 through a diabetic counseling plan, a 2023F code will also populate as a line item on the bill.
This is intended to accommodate a misalignment between the MIPS and HEDIS programs and reduce the burden of reporting diabetic cases for both programs. Starting in 2022, this measure requires evidence of retinopathy. This specification required changes to the Ocular Exam selections and the MIPS reporting in Eyefinity EHR. These changes were implemented in June 2022. For patients seen between January and June 2022, you will need to review the MIPS report and mediate the results as needed.
Document denominator exclusions:
- Open the Ocular Exam.
- Select an exam.
- Select MIPS as the diagnosis and MIPS Quality as the plan.
- Tap the Diabetes tab.
- Select a condition from the smartQuality 117 (Denominator) drop-down and tap Done.
- Document the rest of the exam and Finalize.
Screen the patient for diabetes:
- Open the Ocular Exam.
- Select a dilated eye exam.
- Select a diabetes diagnosis from Impressions.
- Select a counseling Plan. Select the plan options and tap Done.
- Tap Special Plans and select All Tests.
- Select the Fundus Photos plan.
- Document the rest of the exam and Finalize.
This renders favorable only if the patient has retinopathy.
Alternatively, code quality with a special plan:
- Open the Ocular Exam.
- Select the MIPS diagnosis and MIPS Quality plan.
- Tap the Diabetes tab.
- Select the appropriate condition from the smartQuality 117 drop-down.
- Tap Done.
- Document the rest of the exam and Finalize.

- A diagnosis of retinopathy during the measurement period and a retinal or dilated eye exam by an eyecare professional in the measurement period
- Diabetic with no diagnosis of retinopathy during the measurement period and a retinal or dilated eye exam by an eyecare professional in the measurement period or the year prior to the measurement period
The eye exam must be performed or reviewed by an ophthalmologist or optometrist, or there must be evidence that fundus photography results were read by a system that provides an artificial intelligence (AI) interpretation. Alternatively, results may be read by a qualified reading center that operates under the direction of a medical director who is a retinal specialist.
