Performing a Routine Vision Exam from the iPad
You can use the Vision Exam screen to view and edit the findings of a routine visit exam. There are five areas required for billing:
- Visual acuity
- Pupils
- Motility
- Visual fields
- IOP
The tabs of the required areas are marked with an asterisk.
This topic describes how
- Open the Vision Exam screen.

- Tap Wearing (WRx).
The Wearing Prescriptions tab opens.
- Tap Add Eyeglasses.
The Add Eyeglasses Rx screen appears.
- Tap View/Import Refraction Hx, and select the refraction you want to import, or enter the details for the patient’s eyeglasses:
- Refractions. Select the Eyeglass Type and Eyeglass Usage from the drop-down menus. Tap the appropriate Eyeglass Options. Select the values for the Eyeglass Prescription from the drop-down menus.
To reveal additional prescription detail options, tap the down arrow button on the Glasses Prescription tab.
To copy the prescription for the right eye to the left eye, tap Copy OD to OS.
- Visual Acuities. Select the patient’s Distance Vision and Near Vision from the drop-down menus. You can tap the 20/20 OU link to populate the Visual Acuities.
You can tap the down arrows to select the Glare, BAT, PAM, and RAM.
- Refractions. Select the Eyeglass Type and Eyeglass Usage from the drop-down menus. Tap the appropriate Eyeglass Options. Select the values for the Eyeglass Prescription from the drop-down menus.
- Open the Vision Exam screen.

- Tap Auto Refraction.
The Auto Refractions tab opens.
When using the Equipment Integration, the Auto Refractions tab may be automatically populated with readings from the autorefractor entered in the Eyefinity EHR web application.
- If you need to add refraction readings manually, tap Add Refraction.
The Refractions screen appears.
- Tap View/Import Refraction Hx, and select the refraction you want to import, or enter the details for the patient’s refraction.
The available tabs are:
- Refractions. Select the values for the Refraction from the drop-down menus. Select the values for the Additional Refraction Fields.
To copy the prescription for the right eye to the left eye, tap Copy OD to OS.
- Visual Acuities. Select the patient’s Distance Vision and Near Vision from the drop-down menus. You can tap the 20/20 OU link to populate the Visual Acuities. You can tap the down arrow to select the Glare, BAT, PAM, and RAM.
- Refractions. Select the values for the Refraction from the drop-down menus. Select the values for the Additional Refraction Fields.
- Tap Save.
If the Visual Acuities are documented correctly in the Wearing Rx tab, you do not need to document them again in the Visual Acuity tab.
- Open the Vision Exam screen.

- Tap Visual Acuity (VA).
The Visual Acuities tab opens.
- Select the Distance Test Type, Distance Correction Type, Near Test Type, and Near Correction Type.
- Tap Save.
- Open the Vision Exam screen.

- Tap Visual Acuity (VA).
The Visual Acuities tab opens.
- Tap the Infant Vision tab.
- Select the values for the CSM Method from the drop-down menus.
- Tap Save.
- Open the Vision Exam screen.

- Tap Refractions.
- Select the type of refraction you want to add from the drop-down menu.
The Refractions tab appears.
- Tap View/Import Refraction Hx and select the refraction you want to import, or enter the details for the patient’s refraction:
- Refractions. Select the values for the refraction from the drop-down menus. Select the values for the Additional Refraction Fields.
To copy the prescription for the right eye to the left eye, tap Copy OD to OS.
- Visual Acuities. Select the patient’s Distance Vision and Near Vision from the drop-down menus. You can tap the 20/20 OU link to populate the Visual Acuities. You can tap the down arrow to select the Glare, BAT, PAM, and RAM.
- Refractions. Select the values for the refraction from the drop-down menus. Select the values for the Additional Refraction Fields.
- Tap Save.
- Open the Vision Exam screen.

- Tap Refractions.
The Refractions tab appears with a summary of the patient’s refraction information.
- Tap +Final Rx.
The Rx Details tab of the New Glasses Rx screen appears.
- Complete the fields on the following tabs:
- Details. Select the values for the Prescription from the drop-down menus. Enter the prescription Start Date and Expiration Date. Select the Visual Acuity for Distance and Near. Select the Usage.
You must select a Usage or you will not be able to save the prescription.
- Lens Details. These options are not used in Eyefinity EHR.
- Frame Details. These options are not used in Eyefinity EHR.
- Special Instructions. Enter any special instructions.
- Details. Select the values for the Prescription from the drop-down menus. Enter the prescription Start Date and Expiration Date. Select the Visual Acuity for Distance and Near. Select the Usage.
- Tap Save & Send. to save an immediately send this prescription to the practice management system.
OR
Tap the drop-down arrow () next to the Save & Send and select Save to save this prescription so you can send it later.
The Save & Send and Save options are sticky. So, you're always presented with the last option you selected.
To send all contact lens and eyeglass prescriptions at once, select Save, and go to Sending Final Prescriptions from the iPad.
- Open the Vision Exam screen.

- Tap Pupils.
The Pupils tab opens.
- Tap Mark as Normal or select the pupil Measurements and Reactions.
You can tap the Mark Pupils, VF and Motility as Normal to mark each section of the vision exam as Normal.
- Tap Save.
- Open the Vision Exam screen.

- Tap Motility.
The Motility tab opens.
- Select the Motility from the drop-down menu.
- Tap Mark as Normal, or select the test performed:
- Ductions and Versions. Select the values for the right and left eye on the drawing.
- Cover-Uncover. Select the Near and Distance values.
- Alternate cover. Select the Top, Middle and Bottom values.
- Light Reflex. Select the Hirschberg and Krimsky values.
You can also select “Attempted but not achieved.”
You can tap the Mark Pupils, VF and Motility as Normal to mark each section of the vision exam as Normal.
- Tap Save.
- Open the Vision Exam screen.

- Tap Visual Fields.
The Visual Fields tab opens.
The Visual Fields defaults to Confrontation Visual Fields. This is a sticky field. If you change it, your change is retained for the logged in user.
- Tap Mark as Normal, or select the test type and test results.
You can also select “Attempted but not achieved.”
You can tap the Mark Pupils, VF and Motility as Normal to mark each section of the vision exam as Normal.
- Mark any problem areas on the Visual Field drawing.
- Tap Save.
- Open the Vision Exam screen.

- Tap IOP.
The IOP tab opens.
- Enter the IOP readings. You can also select “Attempted but not achieved.”
This is a sticky field. If you change it, your change is retained for the logged in user.
- Tap Save.
- Open the Vision Exam screen.

- Tap Diagnostic Drops.
The Diagnostic Drops tab opens.
- Select the dilation solution used for each eye.
This is a sticky field. If you change it, your change is retained for the logged in user.
- To copy the prescription for the right eye to the left eye, tap Copy OD to OS.
- In Counsel, indicate whether you have counseled the patient.
- Tap Save.
- Open the Vision Exam screen.

Refraction history is available on any tab where a refraction is documented, such as Wearing Rx, Auto-Refraction, Contact Lens, RGP/Hybrid, and Refraction.
- Tap Refractions.
The Refractions tab opens.
- Add a refraction using the drop-down menu.
- Tap View/Import Refraction Hx.
A history of the patient’s refractions appears.
- Tap Close to exit the refraction history.
