Documenting Refractions in the in the Detailed Visit Overview (iPad)
Document refractions in the Vision Exam section of the Detailed Visit Overview. Depending on your selections, Encompass can automatically document the following refractive diagnoses and refraction plan based on the manifest refraction:
- Myopia
- Hyperopia
- Astigmatism
- Presbyopia
- Anisometropia
What do you want to do?

- Open the Vision Exam screen. To learn how, go to Opening the Vision Exam in the Detailed Visit Overview (iPad).
- Tap the Wearing (WRx) tab.

Tap Add Eyeglasses.
The Eyeglasses screen opens. Continue documenting the wearing prescription.

- Add an eyeglass wearing prescription. To learn how, go to Add an eyeglass wearing prescription.
- Select the Eyeglass Type, Eyeglass Usage, and Eyeglass Options as needed.
- Document the Binocular PD Distance and Binocular PD Near as needed.
- Import a previous refraction. To learn how, go to Viewing Refraction History and Importing Refractions in the Detailed Visit Overview (iPad).
OR
Enter the details for the patient’s eyeglasses:
- Select the values for the Eyeglass Prescription from the drop-down menus.
Tap the Manual toggle to type the prescription values manually rather than select the values from drop-down menus.
- To copy the prescription for the right eye to the left eye, tap Copy OD to OS.
- Select the values for the Eyeglass Prescription from the drop-down menus.
- Document the patient's visual acuities as needed:
- Tap the Visual Acuities tab.
- Select the patient’s Distance Vision and Near Vision from the drop-down menus.
Tap the Set to 20/20 OU and Set to J1+OU links to populate the Visual Acuities fields with those values.
- Tap Save.

- Open the Vision Exam screen. To learn how, go to Opening the Vision Exam in the Detailed Visit Overview (iPad).
- Tap the Auto Refraction (ARx) tab.
If your practice uses the Equipment Integration, the Auto Refractions tab may be automatically populated with readings from the autorefractor.
- Tap Add Refraction to add a new refraction.
The Auto Refraction screen opens.
- Import a previous refraction. To learn how, go to Viewing Refraction History and Importing Refractions in the Detailed Visit Overview (iPad).
OR
Enter the refraction details manually:
- Select the refraction values from the drop-down menus.
Tap the Manual toggle to type the prescription values manually rather than select the values from drop-down menus.
Tap Show Refractive Measurements to view the last recorded wearing (glasses and contact lens), autorefraction, manifest, cycloplegic, and keratometry readings. The refractive measurements remain visible, so you can see them as you select current values, until you tap Hide Refractive Measurements.
- To copy the prescription for the right eye to the left eye, tap Copy OD to OS.
- Select the refraction values from the drop-down menus.
- Select an Auto Refraction Type.
- Enter the Binocular PD (distance and near) as needed.
- Document the Additional Refraction Fields, like horizontal prism, vertical prism, base, slab off, vertical decentration, monocular pupillary distance (distance and near), and underlying condition as needed.
- Document the patient's visual acuities as needed:
- Tap the Visual Acuities tab.
- Select the patient’s Distance Vision and Near Vision from the drop-down menus.
Tap the Set to 20/20 OU and Set to J1+OU links to populate the Visual Acuities fields with those values.
- Tap Save.

- Open the Vision Exam screen. To learn how, go to Opening the Vision Exam in the Detailed Visit Overview (iPad).
- Tap the Refractions (MRx/CRx) tab.
- Tap the Add Refraction drop-down and select Manifest Refraction.
The Manifest Refraction screen opens.
- Ensure the Generate Dx option is selected if you want Encompass to automatically create a refractive diagnosis and plan based on the refraction.
- Select a Usage Type.
- Import a previous refraction. To learn how, go to Viewing Refraction History and Importing Refractions in the Detailed Visit Overview (iPad).

Enter the refraction details manually:
- Select the refraction values from the drop-down menus.
Tap the Manual toggle to type the prescription values manually rather than select the values from drop-down menus.

- Tap Save.

- Tap the Refractions (MRx/CRx) tab.
- Tap the Add Refraction drop-down and select Cycloplegic Refraction.
The Cycloplegic Refraction screen opens.
- Select a Usage Type.
- Import a previous refraction. To learn how, go to Viewing Refraction History and Importing Refractions in the Detailed Visit Overview (iPad).

- Tap Save.

- Open the Vision Exam screen. To learn how, go to Opening the Vision Exam in the Detailed Visit Overview (iPad).
- Tap the Refractions (MRx/CRx) tab.
- Tap the Add Refraction drop-down and select Retinoscopy Refraction.
The Retinoscopy Refraction screen opens.
- Select a Usage Type.
- Import a previous refraction. To learn how, go to Viewing Refraction History and Importing Refractions in the Detailed Visit Overview (iPad).

- Select the refraction values from the drop-down menus.

- Tap Save.

There are multiple ways to create a final glasses prescription: create a final prescription from a refraction or add a new final glasses prescription.
- Open the Vision Exam screen. To learn how, go to Opening the Vision Exam in the Detailed Visit Overview (iPad).
- Tap the Refractions (MRx/CRx) tab.
- Locate a suitable refraction and tap + Final Rx.
OR
Tap the Add Refraction drop-down and select Final Glasses Refraction.
The Retinoscopy Refraction screen opens.
- Select a Usage Type.
The prescription is automatically set to Active. The Start Date is based on the current date. The Expiration Date is based on your practice's facility settings.
- Import a previous refraction. To learn how, go to Viewing Refraction History and Importing Refractions in the Detailed Visit Overview (iPad).

Tap the Manual toggle to type the prescription values manually rather than select the values from drop-down menus.
