Creating the Transition of Care (TOC) Document

A TOC document contains all of the information that a CDA does, along with mental health functional status information. Follow these instructions to create a TOC document when a patient is referred or transitions to a new care setting.

  1. Open the patient’s exam in ExamWRITER and click Patient Hx.
  2. Click the Demographics tab and verify the patient’s Name and Date of Birth.
  3. Click the Exam Hx tab and click New Referral.

    The Patient Referral window opens.

  4. Select a Referring Provider and a Refer To Provider.
    Click Maintain to verify that the referred provider has a valid email address recorded.
  5. Select a SNOMED that best explains the reason for the referral.
  6. Select an Expected Return Date and click Save.
  7. Click Create CDA Transition of Care.

    The TOC document is saved as a PDF file in eDocuments.

  8. Click Exit to close the Patient Referral window.
