Modifying Practice Information

  1. Log into the provider portal as a practice administrator. For more information about logging into the provider portal, go to Logging into the Provider Portal.
  2. Click Administration, select Practice Admin, and select Manage Practices.
  3. Click Edit next to a practice name.
  4. Modify the practice information, as needed.
    • Do not change the Office Phone or External Practice ID.
    • Fields marked in red asterisks are required.
    • The practice information appears on clinical health summaries for your patients to see.
    • Modifying the information on this window does not change the information on the Business Names window in OfficeMate. To change the information on the Business Names window, contact Eyefinity at 1.800.942.5353.
    • If you are part of a multilocation practice, modify the information for the home office.
  5. Click Save.
