Verifying User Names & Changing Passwords

The practice administrator user name and password were established when the portal was initially setup with Eyefinity. If you forget your administrator user name and password, contact Eyefinity at 1.800.942.5353.

The provider user names and passwords were established after providers created their first CDA for a patient. If you forget your provider user name and password, ask a practice administrator to log into the portal and verify your user name and change your password.

To change a provider’s password:

  1. Log into the provider portal. For more information, go to Logging into the Provider Portal.
  2. If you are logged in as a practice administrator, follow these instructions; otherwise, go to step 3.
    1. Click Administration and select User Accounts.
    2. Select Clinician from the Access Type drop-down menu.
    3. Search for and select a user. If necessary, note the user’s user name in his or her account.
    4. Go to step 4.
  3. If you are logged in as a provider, click My Account in the upper-right corner of the page.
  4. Scroll down to the User Account Information section.
  5. Type a new password in the Change Password text box and retype it in the Confirm New Password text box.
  6. Do not change the Direct Password.
  7. Click Save.
