You may select activities that match your practice’s goals from a list of more than 100 options.
Small, Rural, and Nonpatient-facing Practices
Small, rural, and nonpatient-facing practices have fewer opportunities to engage in these improvement activities, so they get more points for the activities they complete. Each eligible clinician must complete one of the following for at least 90 days during the calendar year:
- One high activity
- Two medium activities
Large Practices
If your group has more than 15 eligible clinicians, each eligible clinician must complete one of the following for at least 90 days during the calendar year:
- Two high activities
- One high and two medium activities
- Four medium activities
At least 50% of clinicians reporting as a group must perform the same activities for a continuous 90-day period.
CMS requires you to document your completion of improvement activities and retain that documentation for six years following your attestation.
