Completing Intake Forms

Patients can complete portions of their medical history in their patient portal and providers can view this information in the provider portal. For more information on viewing completed intake forms, go to Managing Patient Profiles in the Provider Portal.

  1. Log into the patient portal. For more information, go to Logging into the Patient Portal.
  2. Click Intake Forms in the top navigation bar and select Intake Forms.
  3. Select appropriate history, allergy, and medication information.
  4. Selections are immediately saved.

At this time, any medical history entered in the patient portal does not transfer into ExamWRITER.
