Managing Patient Profiles in the Provider Portal

  1. Log into the provider portal. For more information, go to Logging into the Provider Portal.
  2. Type search criteria into the Search Patient Profile text boxes and click Search.
    The Patient ID in the provider portal is the same as the Patient # or Patient ID in OfficeMate.

    The Account # is assigned by the provider portal.
  3. Click a patient’s name to open the patient’s profile.
  4. Scroll down to the bottom of the page and click Back to Patient Profile.
  5. To view and request external summary of care documents from a third party when the patient transitions to your care or is referred to you from another provider, click View External Documents and follow these instructions:
    1. Click Request Document.
    2. Click Request next to Information Exchange.
    3. Click Close.
    4. Click Back to Patient Profile.
  6. To add a new external reference to the patient’s profile, click +Add New and follow these instructions:
    1. Type a note about the new external reference in the Note text box.
    2. Type the URL to the document.
    3. Click Create.
  7. To view the intake forms that the patient completed in his or her patient portal, click View Intake Forms. For more information, go to Completing Intake Forms.
