Managing Patient Profiles in the Provider Portal
- Log into the provider portal. For more information, go to Logging into the Provider Portal.
- Type search criteria into the Search Patient Profile text boxes and click Search. The Patient ID in the provider portal is the same as the Patient # or Patient ID in OfficeMate.
The Account # is assigned by the provider portal. - Click a patient’s name to open the patient’s profile.
- Scroll down to the bottom of the page and click Back to Patient Profile.
- To view and request external summary of care documents from a third party when the patient transitions to your care or is referred to you from another provider, click View External Documents and follow these instructions:
- Click Request Document.
- Click Request next to Information Exchange.
- Click Close.
- Click Back to Patient Profile.
- To add a new external reference to the patient’s profile, click +Add New and follow these instructions:
- Type a note about the new external reference in the Note text box.
- Type the URL to the document.
- Click Create.
- To view the intake forms that the patient completed in his or her patient portal, click View Intake Forms. For more information, go to Completing Intake Forms.
