Printing a Minor Consent Form

For patients under 18 years old, you can print the Permission to Consent for Medical/Optometric Care to Minor Child or Incapacitated Adult form. The form is not customizable at this time.

  1. Search for the patient. The patient must be less than 18 years old.
  2. Open the patient's Overview or Demographics screen.
  3. Click Minor Consent Form.

    The Permission to Consent for Medical/Optometric Care to Minor Child or Incapacitated Adult form opens in a new window.

    If the form does not open, check to make sure your browser is set to allow pop-up windows. For instructions, see Enabling Pop-ups
  4. Click Print.
  1. Click Appointments and then select Appointment Confirmations from the drop-down list.
  2. If necessary, change the appointment date. For instructions, go to Viewing Appointment Confirmations.
  3. Click Minor Consent Form.

    The Minor Consent Form window opens and displays a list of patients under 18 years old who have appointments on the selected date.

  4. Select the Print check box next to each patient for whom you want to print a form.
  5. Click Print.

    The form opens in another window.

  6. Click Print.
